To notify us about a claim, please use our PDF forms in which all necessary information are requested. You can send your completed form directly to our claims department, email: firstname.lastname@example.org. Please note that we always need a version with your signature to process your claim. You can send it to us scanned by e-mail or fax or by traditional mail.
